Admissions Essay
Copy of Driver’s License (or State ID)
Copy of Social Security Card (or Birth Certificate)
Copy of High School Diploma or G.E.D (Can also be Transcripts)
Application Fee (Non-refundable) - $30.00
Permit Fee - $25.00 (Permit fee for Instructor Program is $50.00)
If you are a transfer or out-of-state student we will need a transcript from your previous school and your hours will need to be verified through the Kentucky State Board.
